This form must be completed by anyone who is not a Central Indiana Soaring Society, Inc. member (dba Soar Indy) prior to flying.

For and in consideration of the agreement of the Central Indiana Soaring Society, Inc. (dba Soar Indy) to the use, by the undersigned Guest, whether as a paying or non-paying pilot or passenger of any aircraft owned, leased or in the custody of the Society in such use, the undersigned hereby releases and discharges Soar Indy and its agents, employees, servants, successors and assignees from any and all liability whatsoever, whether by reason of negligence, gross negligence or otherwise arising from or related to such use, for personal injury or death to the undersigned or anyone else, property damage suffered by the undersigned or anyone else, and ordinary other occurrence whatsoever, and the undersigned further agrees to indemnify and hold Soar Indy harmless, including attorney fees, from any and all liability to anyone else arising from said use.
